Please note we are only in network with Blue Cross Blue Shield PPO, BCBS Blue Advantage HMO (PCP Insurance Referral is required), and United Health Care. (These can't be affiliated with Medicare.)

Our non-insurance private pay flat rate is as follows:

• Initial appt is $170, each additional are $75. (Children 12 and younger are $75 for all appts.)
• Going 2 years between appts requires a re-evaluation at the initial appt cost.
